    It looks like you've cracked open the regulator housing, the part you have indicated by the red arrow, tighten it back up fully shut, looks like you have a pre-set unit, and just us the needle valve to control flow rate.

    the nock down valve on the extinguisher may be sticking, and/or the sound you are hearing may just be residual pressure within the reg, take it outside open the valves as if you were using it in the tank then increase gas flow and allow it to fully vent to atmosphere

    No need to use carbon in a planted tank, yes it will absorb a substantial amount of the ferts, until it becomes saturated and can't absorb any more, also no need to try and chemically remove melafix or primafix  :)
