Something wrong with kuhli loach :(

When cleaning tanks today I noticed one of my kuhli loaches acting funny. Swimming kind of weird abd breathing fast. Once I got a good look at him it appears half of his face is missing!!! I've never seen this before. None of my other fish are acting funny. He's in a tank with mollies and cardinal tetras and one killifish. Water parameters are good. Ammonia 0, nitrite 0, and nitrate 40. Hopefully everyone can see the picture clearly as I'm doing this from my phone.
