Snail Dying?

Hi everyone-
Does anyone know what my snail is doing? Two days ago one of my mystery snails died and today it seems this one is. I've had them for three months and they were fine up until now. I have added anything to the tank.
Water Parameters:

ph-7.8
nitrite- 0 ppm
nitrate- 30 ppm
ammonia- 0 ppm
gh- around 200 ppm
kh-200 ppm

It doesn't look like it. The snail is about 1 1/2 to 2 inches. When I purchased it it was half that size.
 
Does it tip-toe when it moves? It may have an irritated foot. Mine were "hugging" the sand when I first moved them to the tank, so I decided to get them out because they kept curling and not being active. They are now in a tank with Fluval Stratum substrate and seem much happier.
 
That's the problem; it isn't moving around. It almost seems to weak to move itself.
 
Try moving him out of the tank and keep the water dechlorinated, nice and clean, no substrate. Put just a bit of food nearby so there is no effort required and see if it takes it. Keep trying, and I hope it gets better.
