I have a shoal of 9 Rummies one of them has a big white patch on the side.
I know there is some format I have to use can someone post it please because I don't know where to find it.

Tank size: 200L 100cmL x 50cmH x 40cmD
pH: 7.0-7.5
ammonia: 0ppm
nitrite: 0ppm
nitrate: 20 - 40ppm
kH: 4 - 80ppm
gH: 180ppm
tank temp: 24'c

Fish Symptoms (include full description including lesion, color, location, fish behavior):
White mark on side, fish acting normally, shoaling, eating etc.

Volume and Frequency of water changes: 25% Weekly

Chemical Additives or Media in your tank: Fertiliser

Tank inhabitants: See signature.

Recent additions to your tank (living or decoration): 3 Rummies [2 weeks ago], 6 Diamond Tetra [2 Weeks ago], 6 Panda Cories [1 week ago], Cryptocoryne Undulata 'Broad Leaves' and Cryptocoryne x Willisii [Two days ago]

Exposure to chemicals: Dosed Protozin and have used Fertiliser

Digital photo (include if possible): Unable

Does the patch on the fish look like cotton wool?
Check the patch for any red areas?
Does the white patch look like bleaching beneath the skin?
 
The patch does look like its beneath the surface yes.

Bleaching beneath the skin can be columnaris.
Have you noticed any fish flicking and rubbing?

What is your location to suggest a medication?
 
I haven't noticed any flicking or rubbing, and I live in the UK.
