My Plants Keep....

My plants keep going brown and then dying. I have managed to keep one alive but its been there for about a year now but its in the smaller tank. In my bigger tank, the plants go brown, bit come off them and makes the tank look awful. how could I stop my plants from doing this?
The plants are all plants but mostly happens to Amazon Swords. Don't know many plant names but I've tried loadz! the only one it doesn't happen to is elodia but thats pants! lol

how big is your tank?
how much light do you have?
are you using a special plant substrate? if not what are you using?
are you adding any fertilizers?
