ludwigia palustris melting

Help! I have almost lost all my ludwigia palustris.
The new growth are healthy but the old leaves and steam are melting. The steam turns black and decay completely.
 
The light is -

Finnex Stingray 2 Aquarium LED Light​

I have CO2
I have organic potting mix with sand on top as substrate
I also dose with all in one fertilizer and iron every week.
 
Did you recently buy this plant? If so, it could be that the plant was grown out of water, and it is now acclimating to being submerged.
Just a thought.
 
The plant was in bad condition when I bought it. But it’s almost 5 months I have it. Never recovered. Always keeps melting and the top barely survives
