Ich has 3 stages. The first is on the fish, then it falls of in a sort of cyst and the parasite multiplies inside it. Finally the cycst splits open and lots of baby parasites go looking for a fish to infect. Medication can only kill the parasite in the last free swimming stage so there must be some medication in the water when the very last cyst opens.
The instructions will probably say to add the medication on certain days, and to continue adding it until so many days after the last spot disappears.
For future reference in case the fish ever get it again, the best way to treat ich is to raise the water temperature to 86 deg F and leave it at that temp for 2 weeks.
