Help! My tetras are not looking good NTD or columnaris?

I think my tetra tank is now again infected with columnaris or NTD…

My tetra 5 months ago had some disease (NTD or columnaris) but he died and I thought everything was okay UNTIL NOW!

one is tiny and so thin (idk what happened) he has been like this for months now

The other now has a white patch on his side like my other tetra had before he progressively got worse and died a couple months later. I found out yesterday.

I haven’t treated for anything yet but I think I should dose paracleanse?

Help what should I do?????

Temp: 78
Tank:10 gallon 6 total tetras plus snail
Food: fish flakes and bloodworms
No ammonia nor nitrite.

The skinny fish probably has intestinal worms and or gill flukes.
Section 3 of the following link tells you how to treat worms in fish. One of the dewormers (Praziquantel) also treats gill flukes.
