Help Betta Has Severe Fin And Tail Rot!

Okay so my mother in law was taking care of my beta for a month while I was away for work. I come back to half his tail missing and some cloudy stuff coming out of his nose. He still has his energy and appetite, but he looks pretty bad. I put him in a gallon bowl for treatment. Salt water and melafix. I hear so many different opinions on how to treat this. Has anyone treated severe fin rot, and what worked???
 
Housing:
How many gallons is your tank?
Does it have a filter?
Does it have a heater?
What temperature is your tank?
Does your tank have an air stone or other type of aeration?
Does your Betta have tank mates? What kind?
Maintenance:
Before your Betta became ill how often did you perform a water change?
What percentage of water did you change?
Do you vacuum the substrate or just dip out water?
What additives do you use?

Water Parameters:
What are your water parameters? Please give exact numbers. If tested by pet store please get exact numbers. "Fine" or "Safe" won't help us help you.

Nitrate:
pH:
Hardness (GH):
Alkalinity (KH):
 
Can you please provide a photo
 
Also best to keep him in purely clean water for now as the medication and salt can add stress to an already stressed fish. The fin rot will clear up by itself provided the water is clean.
 
This has also happened to me but after a few water changes his tail was normal again, it was very weird.

Fin rot is the direct result of poor water quality.
