Having problem with a couple plants

OK...i have a plated tank that was doin great...
now all of a sudden, i have a few problems.
1- I had beatifull sword plants...then they turned transluscent and yellow.
I gave up and got rid of them.
2- I got a small Anubias barteri round leaf, now i notice that the leaves are turning very very dark in spots
My other plants grow great...but i do notice that the older leaves fall off very fast

According to my LFS, the light is fine (i use a life glo)
I added some fertilizer (which i have never had to use), and the next day i noticed the hair algae on one plant.

I am having a hard time figuring out whats wrong.

What size is your tank? How much light do you have? What type of substrate? What is your light schedule? The answers will help us give you a possible solution to the problem.

Lighting is a Life Glo 15W
Just regular substrate.
