This guppy doesn’t even look that pregnant. Any thoughts on what is coming out of her here? Is it an egg sac?

It’s been like this for at least an hour, maybe more. She’s rubbing her belly on the substrate to get to it.
 
Guppies are livebearers. Apparently some fish get a prolapse, maybe you could research that and see if that would explain it?

Looks like it is a prolapsed uterus and the most humane option is to euthanize:(

It's not a tumour because they take months to grow.

Part of the fish's intestine has popped out of its butt. There's not much you can do except hope it goes back in by itself.
