Tank size: 120 Gallon

nitrite: 0
nitrate: 30
kH: N/A
gH: N/A
tank temp: 79F

Fish Symptoms: No noticeable symptoms, fish are dying at a rapid pace Tuesday I lost a Molly, Wednesday I lost two more Mollies and a Tetra, Today I noticed a Cory dead this morning, and just pulled a dead angel tonight.

Volume and Frequency of water changes: Large (30-70%) daily ending three weeks ago during a large 6 day NO2 spike, then reduced to 20% biweekly.

Chemical Additives or Media in your tank: Pimafix

Tank inhabitants: Angelfish, Cory Cats, Plecos, Mollies, Tetras, Clown Loaches, Stripped Raphael Catfish, Mystery Snails

Recent additions to your tank (living or decoration): Added a lot of live plants Monday, including additional Fril, Swords, and newly added moneywort and scarlet

I had a NO2 spike a few weeks ago which caused me to lose two Corys, I did large water changes daily to combat the effects of the NO2 and it stabilized in about 5-6 days. This was followed closely by a columnaris outbreak so I quarantined the affected fish and treated the tank with half strength Pimafix to prevent additional outbreaks. I treated the tank for 7 days and added carbon back to the filter a week later accompanied by a 10% water change. Everything seemed fine for nearly two weeks, we added a lot of plants on Monday and starting Tuesday we are losing fish daily, water parameters seem fine so I am not sure whats going on. I did notice a dead angelfish today and pulled him out, he was a lot smaller than the remaining angels who were all purchased six weeks ago at the same time, I was sure they were about the same size so perhaps this guy died a long time ago and has been rotting, could that explain the sudden fish die off? If so what can I do? Would a large water change and fresh carbon be enough? This is just a guess, I really have no idea what could be causing the die off and would really appreciate any advice anyone could offer.

Again I do not notice any symptoms but have lost six fish in 72 hours, a molly on day one, two more mollies and a tetra on day two, and a cory and angel today. Water tests are fine, the only recent additions are live plants, totally at a lost as to what could be causing this. Not sure if this is important but figure I will give all information available, the tank is also quite cloudy this started soon after adding the Pimafix and I thought it was because I removed the carbon, however it has not cleared up much if at all since new carbon was installed. Also going to keep a close eye on the Mystery Snails, maybe some of them died and are rotting as well, I noted where they were tonight if they are in the same spot in the morning I'll pull them out. Would dead fish/snails left in the tank be an explanation for this? I know it would be bad if something was left to rot in the tank, but would it kill without symptoms?

EDIT: Well it's morning of the forth day, and I awoke to find another dead Molly and the remaining Red Minor Tetra gone as well. I really don't understand it, the rest of the fish all seem very healthy, but so did the majority of the fish that suddenly died.
