What are the other two fish? They could be stressing him out.

Danio and rainbow shark

One danio on its own will be aggressive, they need shoals of 6. Rainbow sharks are extremely aggressive, and need at least a 3 foot tank. It's likely that the shark has been bullying both fish.

Danio and rainbow shark

You need to rehome or take back these fish, neither are suitable for such a small tank. The danio is too active and needs others to shoal with and the shark will get far too big, around half a foot long, and become aggressive.
